YOUR RIGHT TO A GOOD FAITH ESTIMATE

Under the federal No Surprises Act, if you don't have health insurance or choose not to use it, you have the right to a "Good Faith Estimate" of how much your care will cost before you receive it. Divergent Care Solutions LLC is a private-pay practice and does not bill insurance, so this applies to all therapy clients.

You have the right to receive a Good Faith Estimate of the total expected cost of your services when you schedule or any time you ask. This includes related costs, such as tests, prescription drugs, equipment, and facility fees.

If you schedule at least 3 business days ahead, you should receive your Good Faith Estimate in writing within 1 business day after scheduling. If you schedule at least 10 business days ahead, you should receive it in writing within 3 business days after scheduling. If you ask for an estimate before scheduling, you should receive it in writing within 3 business days after you ask.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Please keep a copy or picture of your Good Faith Estimate and your bill.

Our current therapy fees are $175 for a 55-minute individual session and $200 for a 55-minute family session, with a limited sliding scale of $100 to $140 per session based on income. Your Good Faith Estimate will list your expected sessions and costs. To request one, contact us.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises, email FederalPPDRQuestions@cms.hhs.gov, or call 1-800-985-3059.
